1. BASEBALL PROGRAM

Liability Waiver & Assumption of Risk

Participation in baseball training and related athletic activities involves inherent risks, including but not limited to physical injury, muscle strains, sprains, fractures, collisions with other players, contact with baseballs, bats, equipment, or training surfaces.

I acknowledge that these risks are a natural part of athletic participation. I voluntarily assume all risks associated with participation in baseball instruction, training sessions, conditioning, drills, games, and related activities conducted by Game Changers / Citius Baseball.

I hereby release, waive, discharge, and hold harmless Game Changers, Citius Baseball, their owners, coaches, instructors, employees, and affiliates from any and all claims, injuries, damages, or liabilities arising from participation in baseball activities, except where caused by gross negligence.

I certify that the participant is physically capable of participating and has no medical condition that would prevent safe participation. I grant permission for emergency medical treatment if deemed necessary.

2.BOXING PROGRAM

Boxing Liability Waiver, Assumption of Risk & Medical Consent

I understand that boxing is a contact sport and involves a significantly higher level of physical risk than many other athletic activities. These risks include, but are not limited to, serious injury, concussion, head trauma, broken bones, cuts, bruises, permanent injury, or death.

I acknowledge that participation in boxing training, conditioning, drills, pad work, sparring (if applicable), and related activities is voluntary and undertaken with full knowledge of the risks involved.

I voluntarily assume all risks, known and unknown, associated with boxing participation. I hereby release, waive, discharge, and hold harmless Game Changers, Citius Baseball, their owners, boxing coaches, instructors, employees, and affiliates from any and all claims, injuries, damages, or liabilities arising from participation in boxing activities.

I certify that the participant is physically and medically fit to participate in boxing training and has no condition that would make participation unsafe. I authorize emergency medical treatment if necessary.

I understand that participation may be suspended or terminated if safety rules are not followed.

3. NEUROSCIENCE PROGRAM

Participation Consent & Media Release

The Neuroscience Program is an educational and instructional program focused on mental performance, cognitive development, learning strategies, and personal growth. While no physical athletic activity is required, participation may include discussions, exercises, or activities designed to support learning and development.

I consent to participation in the Neuroscience Program and understand that it does not replace medical, psychological, or professional advice. I release Game Changers, Citius Baseball, and their instructors from liability related to participation, except in cases of gross negligence.

I acknowledge that photos, videos, or recordings may be taken during sessions for educational or promotional purposes and grant permission for their use unless I notify Game Changers in writing.

GENERAL TERMS (APPLIES TO ALL PROGRAMS)

This agreement shall be governed by the laws of the State of Florida.
I understand that by agreeing, I am giving up certain legal rights.
I confirm that I have read and understand the applicable waiver section related to the program selected.
